*Assessments are required prior to establishing a membership.
There is a $150 application fee. This fee is waived for first time, new patients. For patients who have left the practice (cancelled membership) and wish to return, the $150 fee will be assessed at that time.
Primary Care patients will not be prescribed any high risk medications. A patient may change to a higher level of care at this practice (Addiction Treatment Program or Chronic Pain Recovery) upon doctor’s diagnosis and patient agreement to the program policy.

Refund Policy: If a patient signs up for a membership and pays for the first month Dr. Helman assumes responsiblility for the patient even if they don’t choose to use the services provided. No refund will be issued to the patient as Dr. Helman will continue to be available for the patient on an emergency basis for 30 days after the patient chooses to leave the practice. If a patient signs up for a membership and pays for the first month and chooses to use our services but then decides halfway through the month to leave the practice, again, no prorated refund will be issued. Cancellation of membership after first month requires a 30 day notice.
